What do I need to bring with me?

• CDL*

• Old Medical Card (if you have it)

• Any Doctor notes you may have for any conditions. If you do not bring this, you may be declined your medical card until you can provide the doctor notes*

• If you have to use a sleep-machine (CPAP), please bring the readings from your machine so our Medical Card Doctor can evaluate the readings*

• Medical card forms. Click here for forms. (can be filled out in office)

Can you send the Medical Card to the DMV for me?
